Key Responsibilities- Engage as a key member of the ED/CDU team, offering specialized social work insights in response to referrals and identifying high-need patient populations.
- Conduct thorough psychosocial evaluations of patients, recognizing their needs and formulating a detailed care plan, ensuring all documentation meets regulatory standards.
- Support patients and their families in developing coping strategies for various health challenges.
- Provide consultation on cases involving complex issues such as abuse, neglect, and legal decision-making.
- Facilitate connections to community resources for ongoing support after discharge.
- Utilize theoretical knowledge and practical skills to promote positive changes in patient behavior and emotional well-being.
- Contribute to discharge planning by assessing patient choices and educating them on potential outcomes.
- Coordinate discharge plans to ensure appropriate placement in community settings.
- Offer advocacy, mediation, and educational services to patients and their support networks.
- Participate in ongoing professional development and quality improvement initiatives within the department.
Experience: Minimum of one year in a health care setting.
Education: Master’s degree in Social Work.
Licensure: Licensed Clinical Social Worker or Licensed Social Worker in Hawaii; Basic Life Support certification within 0.5 months of hire.
Additional Skills:- Experience working within interdisciplinary teams.
- Familiarity with medical terminology and the psychosocial implications of serious health conditions.
- Ability to effectively communicate with medical professionals and patients.
- Knowledge of care processes in various health care settings.
- Understanding of patient education principles and resource utilization.

- Awareness of community resources and family dynamics.
- Proficient typing and documentation skills.
