Kaiser Permanente is seeking a highly skilled Licensed Clinical Social Worker to join our team in Honolulu, Hawaii.
Job SummaryThis is a full-time position that requires a Master's degree in social work from an accredited college or university, a passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.
Key Responsibilities- Provides comprehensive case management and ambulatory behavioral health services, including assessment, evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention for members in the outpatient setting.
- Conducts social work consultation, performs clinical diagnosis or psychotherapy, and provides psychotherapy and counseling for individuals utilizing a wide range of evidence-based interventions and modalities.
- Interviews members and evaluates their psychological and mental status, utilizing the latest Diagnostic and Statistical Manual of Mental Disorders (DSM) to diagnose patients.
- Coordinates referral of patients to psychiatrists or other clinicians, assists with admission to hospitals or other specialized facilities, and plans and provides services in the appropriate level of care.
- Assesses treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders, and conducts preparation and evaluation of psychosocial assessments and development of social work service plans.
- Helps patients enhance or restore their capacities for personal and social functioning and preventing and controlling social problems.
- Performs case management, coordination, casework intervention, and monitoring of psychosocial care plans in areas of personal, social, or economic resources, conditions, or problems.
- Applies case management concepts, principles, and strategies in the development of an individualized case plan that addresses the members' broad spectrum of needs.
- Gathers information and identifies resource and referral services for patient care, educates clients regarding behavioral health services, and assists members to use services.
- Provides information, resource identification, and referral services, advocacy services, education, and counseling, and participates in or assists with therapeutic and educational group programs.
- Collaborates and consults with physicians and other interdisciplinary clinical team members to support assessments and decision making.
- Maintains high-level clinical expertise, is knowledgeable of current clinical conditions, methods, medications, theoretical frameworks, and interventions, and utilizes best practices in delivery of behavioral health services in accordance with professional and clinical guidelines.
- Maintains clinical records by documenting clinical assessments, treatment plans, interventions, and all pertinent information in patient medical records according to Regional and departmental guidelines.
- Participates in departmental functions, such as staff meetings, staff in-services, committees, clinical team meetings, departmental planning, and quality management activities as assigned.
- Maintains database and case management tools to monitor status and care gaps of members, participates in orientation of new personnel, and training of staff.
- Assesses patients in Emergency Departments on an as-needed basis in service areas where Kaiser Permanente has privileges to provide mental health assessments and crisis intervention.
- Participates in performance improvement, implements Quality Improvement processes, assists with development and implementation of departments' annual work-plan, performs clinical record audits, and maintains educational record and provides evidence of competency.
- Masters Degree in social work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.
