Position OverviewThe Social Worker will play a crucial role as part of the emergency department/CDU care team, delivering psychosocial evaluations and implementing tailored social medicine services to address the diverse needs of patients and their families.
Key Responsibilities- Actively engage as a member of the ED/CDU care team, providing social medicine insights in response to referrals and identifying high-need populations.
- Conduct comprehensive psychosocial evaluations of patients in the ED/CDU, identifying needs and formulating a plan of care while ensuring compliance with documentation standards.
- Support patients and their families with coping strategies related to health challenges upon referral from the ED/CDU.
- Facilitate social medicine consultations for cases involving abuse, neglect, and legal decision-making.
- Provide information and connect patients with community resources for support following discharge.
- Utilize theoretical knowledge and practical skills to influence positive changes in patient behavior and emotional well-being.
- Contribute to discharge planning by assessing patient choices and educating them on the implications of their decisions.
- Assist in coordinating discharge plans to ensure appropriate placement in community-based settings.
- Offer advocacy, mediation, and educational services to patients and their support systems.
- Engage in ongoing professional development to stay updated on trends in medical social work and participate in quality improvement initiatives.
Experience: Minimum of one (1) year in a healthcare environment.
Education: Master’s degree in Social Work.
Licensure: Must possess a Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii) certification, along with Basic Life Support certification from the American Heart Association within 0.5 months of hire.
Additional Skills- Experience working within interdisciplinary teams.
- Familiarity with medical terminology and the psychosocial effects of serious health conditions.
- Ability to effectively communicate and collaborate with healthcare professionals and patients.
- Knowledge of care processes in various healthcare settings.
- Strong customer service orientation.
- Understanding of community resources and family dynamics.
- Proficient typing and documentation skills.
