About the Role:
Kaiser Permanente is seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team in a Mental Health Clinic setting. As a key member of our behavioral health team, you will be responsible for providing high-quality, patient-centered care to individuals, families, and groups.
Key Responsibilities:
- Provide ambulatory behavioral health services, including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention.
- Assist with the administration, development, implementation, and evaluation of social work programs and policies.
- Work as part of a team in the Call Center Advice Line to provide telephonic triage and assessment for members requesting behavioral health services.
- Provide telephonic crisis management for members seeking behavioral health services.
- Conduct clinical social work consultation, perform clinical diagnosis, risk assessments, and crisis interventions.
- Determine if members are in crisis, at-risk, or suicidal/homicidal and refer them to the appropriate level of care.
- Assess urgent or high-risk clinical situations and make appropriate interventions.
- Assist with admission to hospital or other specialized facilities.
- Plan and provide services in the appropriate level of care.
- Assess needs for mental health treatment and make appropriate referrals as needed.
- Perform case management, coordination, and monitoring of social work service plans in areas of personal, social, or economic resources, conditions, or problems.
- Assess treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders.
- Conduct preparation and evaluation of psychosocial assessments and development of social work service plans.
- Help patients enhance or restore their capacities for personal and social functioning and prevent and control social problems.
- Gather information and identify resource and referral services for patient care.
- Educate clients regarding behavioral health services.
- Research, collect, and record data and information relating to identification and treatment of behavioral and interpersonal problems of patients.
- Review difficult cases or quality assurance concerns with the clinical team.
- Utilize best practices in delivery of behavioral health services in accordance with professional and clinical guidelines.
- Maintain clinical records by documenting clinical assessments, treatment plans, interventions, and all pertinent information in patient medical records according to Regional and departmental guidelines.
- Participate in clinical team meetings, departmental planning, and quality management activities.
- Assess patients in Emergency Departments on an as-needed basis in service areas where Kaiser Permanente has privileges to provide mental health assessments and crisis intervention.
Requirements:
- Masters Degree in Social Work from an accredited college or university.
- Passing score on the clinical level national board examination.
- Supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.
- Demonstrated knowledge of and skill in adaptability, change management, conflict resolution, decision making, group process facilitation, influence, interpersonal relations, oral communication, problem solving, quality management, teamwork, and written communication.
Preferred Qualifications:
- Experience in providing behavioral health services in a primary care setting.
- Experience in providing Consultation/Liaison Services to emergency departments.
- Experience in providing evidence-based treatment, including conducting individual and group therapy, intensive outpatient treatment, and/or partial hospitalization programs for children and/or adults, including people with serious/chronic mental illness.
- Experience in writing treatment plans.
