Position Title: Outreach Specialist
Location: Wellness Center
ROLE SUMMARY
The Outreach Specialist is responsible for connecting members of the LGBTQ+ community with essential health services and case management support. This role involves proactive engagement with community members to foster trust and understanding. The Outreach Specialist will participate in various community events and gatherings to assess the needs and concerns of the population served. Additionally, they will create and implement social media initiatives aimed at raising awareness about mental health, risk reduction strategies, and prevention options related to substance use, hepatitis, and HIV.
KEY RESPONSIBILITIES:
- Deliver educational resources and assist with benefits navigation for priority populations.
- Conduct Rapid HIV/DBS/HCV testing.
- Build and maintain partnerships within the community.
- Develop and execute social media marketing strategies targeting priority populations.
- Facilitate access to PrEP, PEP, IART/HIV or STI treatment, and address Social Determinants of Health.
- Engage in a minimum of three outreach events weekly.
- Keep comprehensive records of interactions, interventions, and outcomes.
- Compile reports to evaluate progress, measure success, and guide future outreach efforts.
- Oversee daily operations at the Drop-in Center, promoting social, educational, and community-building activities.
- Co-facilitate Evidence-Based Interventions and support groups.
- Ensure timely submission of all collected data and reports.
- Meet at least 85% of program deliverables.
- Participate in rotating weekend duties as required.
- Other related tasks may be assigned as necessary.
- High School Diploma or GED, along with relevant practical experience.
- Associate's Degree and relevant practical experience is preferred.
- At least two years of experience in similar roles involving group facilitation and community outreach.
- Strong commitment and understanding of issues related to homelessness, mental health, and substance misuse.
- Experience working with high-risk LGBTQ+ and HIV+ populations.
- Exceptional writing and communication skills.
- Certification in HIV/HCV testing or willingness to obtain certification within 60 days of hire.
- Personal experience within the LGBTQ community.
- Proficient in Microsoft Office Suite and related software.
- Familiarity with AIRS database is a plus.
