Job Title: Community Outreach Specialist

FlexStaff Careers is seeking a highly motivated and experienced Community Outreach Specialist to join our team. As a key member of our organization, you will play a vital role in driving Program of All-inclusive Care for the Elderly (PACE) growth through referral source network building, developing lead-optimization strategies, and helping develop community engagement events to proliferate increased awareness, advocacy, and strategic partnerships with referral sources.

Key Responsibilities:
  • Conduct sales and networking to drive PACE growth through referral source network building
  • Develop lead-optimization strategies and plan education
  • Help develop community engagement events to increase awareness, advocacy, and strategic partnerships with referral sources
  • Focus on outreach to senior centers, senior housing, community-based organizations, community leaders, faith-based organizations, physician groups/IPAs, social workers, and discharge planners surrounding our PACE centers and Alternative Care Sites (ACS)
  • Coordinate and attend events in the community, host events at our PACE centers/ACS, and join network groups to create engagement opportunities with referral sources, prospective enrollees, and their caregivers
  • Integrate with the Intake/Enrollment, Provider Relations, and Marketing Teams to ensure all referrals and enrollments are processed in a timely manner and that all activities/events are compliant with state, federal, and PACE regulatory requirements
  • Maintain ethical conduct and be responsible for the quality and satisfaction of prospective participants, caregivers, and referral sources
Requirements:
  • Associate degree required
  • Minimum of two (2+) years of work experience in healthcare business development; preferably with MLTC, MAP, or PACE programs and/or homecare
  • Knowledge of community-based long-term care services to a dual-eligible and frail adult population
  • Strong interpersonal skills (written and verbal)
  • Experience utilizing CRM or other database platforms/tools preferred
  • Proficiency in Microsoft Word and Outlook required; Microsoft Excel preferred
  • Knowledge of PACE program benefits a plus
  • Highly organized and flexible to meet submission deadlines
  • Experience in working independently without significant oversight and/or direction from a supervisor
  • Written and verbal fluency in a secondary language required (Chinese - Mandarin & Cantonese)
  • Must be a skilled public speaker, willing and comfortable to present in front of audiences
  • Comprehensive understanding of federal and state regulatory requirements with respect to health care eligibility, coverage, and services of various public programs a plus
  • Familiarity with healthcare organizations and general population dynamics within the greater New York Metropolitan region is a plus
  • Willingness to travel throughout the greater NYC metropolitan region (80% of the time)
  • Positive attitude and outlook with the ability to multi-task in a constantly changing environment

Salary Range: $24-$24/hour
