Job Summary

We are seeking a skilled Health Outreach Specialist to join our team at Services For The Underserved, Inc. The ideal candidate will have experience working with children and families, providing direct services and support to meet their individual needs.

Key Responsibilities
  • Provide direct services to children and families, including crisis intervention and support
  • Engage in collateral visits with health professionals and make necessary referrals
  • Coordinate activities to meet the individual needs of children, including infant stimulation and after-school programming
  • Attend staff meetings, in-service training, and weekly clinical meetings
  • Complete progress notes, treatment planning, and other required documentation
Requirements
  • Experience working with children and families
  • Ability to work evenings and weekends
  • Knowledge of substance use and mental illness
  • Computer literacy skills, including Excel and Teams
  • Bilingual English/Spanish speaking preferred
