Job Summary:
We are seeking a highly skilled and dedicated Lead Physical Nutritional Management Plan Coordinator to join our team at the Texas Department of Aging & Disability Services. As a key member of our Habilitation Therapy team, you will be responsible for providing oversight and guidance to direct care staff in the areas of physical and nutritional management, oral and enteral eating, positioning, mobility, communication, and other related PNMP services.

Key Responsibilities:
• Conduct competency-based training of staff and mentor new staff in the implementation of planned client therapy programs.
• Monitor therapeutic activities, communicate problems and recommendations for improvement to Habilitation Therapy staff.
• Ensure the availability, condition, and proper use of assistive equipment and maintain accurate records of PNMP information.
• Participate in PNMP meetings and clinics, NMT meetings, and PST/HST meetings to ensure effective communication and collaboration among team members.
• Maintain client records, ensure proper inventory levels, and complete data system entry in a timely manner.
• Provide new employee orientation and refresher training as indicated, and oversee the training of new employees in topics such as lifting, mealtime procedures, and other PNMP-related activities/plans.
• Perform other duties as assigned, including actively participating in disaster response and/or recovery or Continuity of Operations (COOP) activation.

Requirements:
• Knowledge of rehabilitative programs for individuals with developmental disabilities.
• Knowledge of physical nutritional management plans, oral and enteral eating, positioning, mobility, communication, and other related PNMP services.
• Skill in the use of computers for data entry and training and coaching others to perform job duties.
• Ability to administer first aid or cardiopulmonary resuscitation (CPR) once trained to do so.
• Ability to express ideas in writing with attention to spelling, grammar, and organization, and to follow a series of oral directions.
• Ability to establish and maintain rapport with co-workers and individuals served, and to collect, organize, and enter information into databases.
