Certified Speech Therapist - Senior Care Specialist

Combining Speech Therapy with Care Coordination
We are seeking dedicated Speech Therapists to join our innovative care coordination initiative. This role allows you to utilize your full skill set while engaging in care coordination tasks that enhance your professional journey. Key Responsibilities:
As a Speech Therapist, you will collaborate with a senior living community that may include various care environments such as independent living, assisted living, and memory care. Therapy services are typically funded through private insurance or Medicare Part B.

Your responsibilities will include:

  • Delivering speech therapy and wellness services tailored for older adults.
  • Conducting evaluations, developing treatment plans, and implementing therapeutic interventions as part of a multidisciplinary team.
  • Facilitating care coordination by performing regular wellness check-ins with patients, liaising with healthcare providers and caregivers, and assisting patients with telehealth appointments.
  • Upholding professional standards and ethical practices in line with company policies and clinical guidelines.
  • Contributing to program development, quality enhancement, and problem-solving initiatives.
  • Enhancing each patient's functional well-being and overall satisfaction.

Your Work Schedule:
Our facilities generally operate from 8 a.m. to 5 p.m., Monday through Friday; however, our flexible scheduling allows you to create a work timetable that aligns with your lifestyle.

Qualifications:
To thrive in this role, you should possess:

  • A degree from an accredited Speech-Language Pathology program (New Graduates are welcome).
  • Active ASHA certification and current state licensure in Speech-Language Pathology.
  • Excellent verbal and written communication abilities.
  • A results-oriented approach complemented by critical thinking skills.
  • Proficiency with technology; quick learners are highly valued.
