Reports To: SH Program Manager

FLSA Status: Non-Exempt

Work Schedule:

Monday – Friday, 8:30 AM – 4:30 PM. Flexibility for evenings and weekends is required.

Position Summary:

The Social Services Coordinator is tasked with overseeing a designated caseload of individuals experiencing long-term mental health challenges, substance dependency, and chronic homelessness. The Coordinator evaluates client requirements, formulates, executes, and assesses service strategies, and collaborates with various community resources to fulfill client service objectives.

Key Responsibilities:

  • Establish rapport with clients through the development of trusting and professional relationships.
  • Facilitate client intake procedures as instructed.
  • Collaborate with clients to create personalized service plans with specific, measurable outcomes.
  • Supervise the creation, execution, coordination, and evaluation of the service plan.
  • Provide case management, crisis intervention, assistance, and education in daily living activities as needed, aiming for family unit stabilization.
  • Ensure consistent face-to-face and collateral interactions with clients, service providers, family members, and other team members.
  • Mediate disputes involving providers, family members, clients, and the agency, based on client needs.
  • Develop and maintain knowledge of community resources and referral processes essential for accessing these services.
  • Advocate for and assist clients in obtaining services identified during the planning phase.
  • Follow established discharge protocols when a client is released, ensuring accurate recordkeeping and referrals.
  • Provide transportation resources such as medical cab services or bus tickets and transport clients when necessary.
  • Organize and lead group sessions for clients on various topics that support their health and human service needs.
  • Engage in community networking and outreach.
  • Maintain comprehensive and current client records to ensure compliance with agency standards.
  • Participate in ongoing professional development activities.
  • Adhere to all policies and procedures set forth by Mercy Housing & Shelter Corp.
  • Perform additional tasks as directed to support the agency's mission.

  • Comply with all relevant regulations, laws, and policies/procedures, and understand your job responsibilities.
  • Submit required documentation and meet deadlines as assigned.

Required Skills and Qualifications:

Strong documentation skills, including proficient oral and written communication, computer literacy, timely and accurate record-keeping, critical thinking abilities, and the capacity to motivate clients.

Education and Experience:

  • A Bachelor's degree (B.A.) from an accredited four-year institution or two to three years of relevant experience and/or training; or an equivalent combination of education and experience, preferably with homeless individuals, those with mental health issues, and/or substance abuse.
  • Bilingual candidates, particularly Spanish/English, are encouraged to apply.

Licenses and Certifications:

  • A valid driver's license and a clean driving record, along with proof of automobile insurance, are required.

Compensation and Benefits:

  • Hourly wage ranges from $23 to $26, depending on experience.
  • Community Housing Advocates offers a comprehensive employee benefits package, including medical, dental, retirement plans, short- and long-term disability, and tuition reimbursement. Full-time employees receive 15 days of paid time off annually, in addition to 12 paid holidays.
