The RN Care Coordinator is responsible for providing case management and education to patients with chronic diseases. This role involves offering guidance and resources to patients throughout their care journey, including inpatient settings, outpatient areas, and potentially in-home care when necessary.
The RN Care Coordinator collaborates with medical and allied health professionals in the community to ensure comprehensive care for patients with chronic illnesses. Additionally, this position serves as a valuable resource for primary and specialty physicians in identifying and managing high-cost and high-risk patients.
Minimum Qualifications- Bachelor's Degree in Nursing and a valid nursing license in the State of Connecticut.
- A minimum of 7 years of experience in acute care, home care, or long-term care.
- Certification in a nursing specialty.
- Advanced knowledge of the healthcare delivery system and chronic care populations.
- Strong organizational, communication, problem-solving, and decision-making skills.
- Specialty certification in COPD preferred, along with a minimum of 5 years of experience in managing COPD.
- Master of Science in Nursing.
- Experience in Managed Care.
- Experience in Case Management.
