Medical Records Specialist

Job Summary: We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at Stonebrook Health and Rehabilitation. As a key member of our nursing staff, you will provide high-quality patient care and services in accordance with all regulations and policies.
Responsibilities:
- Provide and oversee patient care and services safely and in accordance with all regulations and policies.
- Currently registered and licensed in good standing with the State Board of Nursing.
- ~2+ years' experience in long-term care preferred.
- ~CPR and First Aid certified.
- Observe, report, and document/charting on patient conditions.
- Set up treatment trays, prepare instruments and/or other equipment, as needed for treatments.
- Review reports from previous shifts to determine changes in status of residents and assign various duties to C.A.'s or other nursing personnel.
- Turn over accurate records and ensure oncoming shift nurses are updated.
- Responsible for notifying Director of Nursing Services (DNS) and/or Administrator of changes in patient condition or other job-related problems.

Requirements:
