Position Title: Healthcare Quality and Risk Supervisor

Department: Administration

Shift: Full-Time

Work Arrangement: Hybrid

Mission: Heartland Alliance Health is dedicated to enhancing healthcare for the most underserved populations, particularly those facing homelessness, mental health challenges, or multiple chronic conditions.

Role Summary: The Healthcare Quality and Risk Supervisor plays a crucial role in spearheading initiatives aimed at developing Population Health Management and Continuous Process Improvement. This position is essential in fostering a culture of excellence across all programs, contributing to clinical quality, patient safety, and performance outcomes through quality assurance and improvement strategies. The Supervisor will implement and manage the Quality Management Program, collaborating with various stakeholders to enhance population health across the organization.

Key Responsibilities:

  • Lead efforts to identify, report, and address quality concerns throughout the organization.
  • Oversee quality management and improvement practices to enhance outcomes at a population level while adhering to accreditation standards.
  • Work closely with clinical leadership to pinpoint areas needing enhancement and develop effective quality improvement processes.
  • Establish and assess performance metrics to evaluate compliance with quality incentives and overall organizational success.
  • Monitor provider education and assist in closing care gaps related to quality measures.
  • Facilitate quarterly peer reviews and collaborate on quality improvement work plans.
  • Participate in licensing, grant funding, and accreditation processes, including site reviews and audits.
  • Lead preparations for accreditation and ensure integration of quality processes into operations.

Committee Leadership:

  • Chair the quarterly quality and risk management committee.
  • Prepare and present quality improvement and risk management reports to senior leadership.

Risk Management:

  • Serve as the health center's risk manager, overseeing all risk management activities.
  • Conduct annual risk assessments and develop action plans based on findings.
  • Analyze data from grievances and incidents to identify improvement opportunities.

Qualifications:

  • Bachelor's degree in Nursing or related field with experience in care coordination and quality management.
  • Experience in Medicaid and managed care settings preferred.
  • Minimum of one year in a healthcare quality role.
  • Proficient in Microsoft Office applications.

  • Effective communication skills, both written and verbal.
