Community Health Navigator Position Overview

We are seeking a compassionate and skilled Community Health Navigator to join our team at OneOncology.

This is a full-time, private practice position that requires 5+ years of experience working as an APP in the Hematology setting. As a Community Health Navigator, you will work with patients in their local communities, providing expert guidance and support to help them navigate the complexities of cancer care.

Key Responsibilities:

  • Develop and maintain relationships with community partners, hospitals, and healthcare providers to enhance patient access to care
  • Provide education and support to patients and their families, addressing emotional and psychological needs
  • Coordinate patient transportation and accommodations, ensuring seamless access to treatment
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of care

Salary: $130,000 - $160,000 per year
