Job Summary:

We are seeking a highly motivated Community Health Support Specialist to join our team at Community Options. As a key member of our support staff, you will play a vital role in ensuring the well-being and happiness of individuals with disabilities.

Responsibilities:
  • Provide direct support to individuals with disabilities, promoting independence and self-advocacy.
  • Develop and implement personalized support plans in collaboration with healthcare providers and family members.
  • Facilitate access to community resources and services, ensuring seamless support delivery.
  • Maintain accurate and timely records of support services and progress.
Requirements:
  • High school diploma or equivalent.
  • Minimum two years of experience in support services, preferably in a community-based setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Estimated Salary:

$30,000 - $40,000 per year, depending on experience.
