National healthcare navigator coordinating care delivery for adults with disabilities in residential programs.

Responsibilities:
Coordinate healthcare services for adults with disabilities in residential settings
Oversee general health issues for individuals with autism in residential or day programs
Monitor medication and medical records for individuals receiving services
Consult with medical professionals to ensure optimal treatments are provided
Supervise DSP staff administration of medication
Maintain accurate medical records
Attend team meetings as scheduled
Work collaboratively with interdisciplinary teams to achieve positive health outcomes
Provide guidance and training to direct care staff on proper healthcare delivery

Requirements:
1-2 years RN experience preferred
Valid NYS RN license required
Associates Degree in Nursing required
Knowledge of OPWDD regulations preferred
BLS, CPR certification, SCIP-R certification required
Ability to assist lifting individuals and 20 lb items
Strong organizational and interpersonal skills
Must maintain individual and family confidentiality

What We Offer:
A competitive salary range: $80,000 - $110,000 per year
