Key Responsibilities- Participate in classroom learning activities, including topics such as critical thinking, evidence-based best practice assessments, care planning, and evaluation of care
- Shadow RNs to gain field experience and review experiences in group debriefs
- Practice in the community with a preceptor as part of an interdisciplinary care team
- Deliver personalized nursing and care management to patients in their home or care facilities
- Constantly evaluate evolving patient needs and respond with plan of care adjustments
- Achieve continued professional growth through attendance at in-services, training, continued formal education, and select journal readership
- Recently received RN license
- Degree in Nursing from an approved program required
- No prior work experience in nursing required
- Must be able to walk up and down steps to access patient residences
