Job SummaryWe are seeking a highly skilled Patient Care Technician I to join our team. The successful candidate will be responsible for providing exceptional patient care and support, while working closely with our research team to ensure the success of our clinical trials.
Key Responsibilities- Assess and Identify Patient Needs: Conduct thorough assessments of patients' psychosocial needs and identify areas for case management support.
- Discharge Planning and Coordination: Assist patients with discharge planning, including coordinating transportation and ensuring all necessary information is accurate.
- Client Support and Advocacy: Provide emotional support and advocacy for patients, while also connecting them with community resources and services.
- Documentation and Record-Keeping: Maintain accurate and up-to-date records of patient interactions, including discharge summaries and case management notes.
- Team Collaboration and Communication: Work closely with our research team to ensure seamless communication and collaboration, while also providing exceptional patient care.
- Education and Experience: High school diploma or equivalent required; 6+ months of experience in a healthcare or clinical research setting preferred.
- Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to effectively interact with patients, staff, and external partners.
- Organizational and Time Management Skills: Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Flexibility and Adaptability: Ability to work a flexible schedule, including rotating weekends and holidays, while also being adaptable to changing priorities and circumstances.
