Job Summary

We are seeking a highly skilled Behavioral Health Specialist to join our team at Delta-T Group Inc. as a Social Worker. As a key member of our team, you will be responsible for providing expert support to youth and families in a dynamic and fast-paced environment.

Key Responsibilities
  • Take a leadership position in supporting program structure and all youth within the program.
  • Demonstrate initiative in supporting and empowering the youth and families.
  • Ensure accurate and secure client files, records, and Individual Service Plans.
  • Participate in live supervision with clients and their families.
  • Meet regularly with the Program Manager for supervision.
  • Demonstrate engagement and leadership in all weekly staff meetings.
  • Assist clients with discharge and aftercare planning as they transition into their next step.
  • Provide timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention.
  • Oversee and review client assessments, provide case management services, and oversee the development of individual treatment plans for clients and families.
Requirements
  • Master's Degree in Social Work (MSW) required.
  • 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults.
  • Expert knowledge of issues surrounding homelessness.
  • Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects.
