We are seeking a highly skilled Behavioral Health Specialist to join our team at Delta-T Group Inc. as a Social Worker. As a key member of our team, you will be responsible for providing expert support to youth and families in a dynamic and fast-paced environment.
Key Responsibilities- Take a leadership position in supporting program structure and all youth within the program.
- Demonstrate initiative in supporting and empowering the youth and families.
- Ensure accurate and secure client files, records, and Individual Service Plans.
- Participate in live supervision with clients and their families.
- Meet regularly with the Program Manager for supervision.
- Demonstrate engagement and leadership in all weekly staff meetings.
- Assist clients with discharge and aftercare planning as they transition into their next step.
- Provide timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention.
- Oversee and review client assessments, provide case management services, and oversee the development of individual treatment plans for clients and families.
- Master's Degree in Social Work (MSW) required.
- 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults.
- Expert knowledge of issues surrounding homelessness.
- Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects.
