Job Summary

We are seeking a highly skilled and experienced RN Clinical Resource Team Leader to join our team at DCH Health System. As a key member of our clinical team, you will be responsible for planning, coordinating, and directing quality, compassionate care to our patients and families.

Key Responsibilities
  • Assess patient condition on admission and ongoing basis, including physical, psychological, social, spiritual, and cultural needs.
  • Plan, provide, and document patient education based on individual needs and barriers to learning.
  • Evaluate and reevaluate patient care, coordinating with patient, family, providers, and other disciplines to achieve patient-centered outcomes.
  • Assign and delegate nursing care to team members based on patient needs and capabilities.
  • Apply safety principles and accurately document observations, care provided, and changes in care plan.
  • Participate in transition rounds, care planning, and team shift reports.
  • Assist in performance improvement activities, including serving on PI teams and implementing changes for improvement.
  • Perform initial and annual competency per job class.
Requirements
  • Current Alabama RN license required.
  • Two years of RN experience preferred, with at least one year of experience in the hospital setting.
  • Current American Heart Association (AHA) Basic Life Support (BLS) Provider training required.
  • Advanced Cardiac Life Support (ACLS) preferred.
  • Ability to read, write legibly, speak, and comprehend English.

Physical Factors
  • Medium work, exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
  • Must be able to tolerate prolonged periods of standing and walking.
  • Must be able to reach reasonable distances in any direction.
  • Must be able to stand, walk, kneel, bend, sit, and stoop.
  • Must be able to run in an emergency.
  • Must have the ability to exert maximum muscle force to lift, push, pull, and carry objects.
  • Must be able to move fingers in a coordinated manner and must be able to feel and perceive temperature, texture, shape, and size with fingertips.
  • Must be able to perform the duties with or without reasonable accommodation.
  • Must possess stamina to work prolonged shifts.
  • Hearing and vision must be normal or corrected to within normal range.
  • Physical presence onsite is essential.
