Job Summary

Heritage Oaks Hospice & Palliative Care is seeking a skilled Hospice Community Liaison to join our team. As a key member of our business development team, you will be responsible for building and maintaining relationships with healthcare professionals, identifying new business opportunities, and driving growth for our organization.

Key Responsibilities:

  • Develop and maintain a strong network of healthcare professionals, including physicians, social workers, and case managers.
  • Identify and pursue new business opportunities, including referrals from hospitals, long-term care facilities, and physician offices.
  • Collaborate with our clinical team to ensure seamless patient care and follow-up support.
  • Conduct regular outreach and education to healthcare professionals, highlighting the benefits of our hospice services.
  • Monitor and analyze market trends, competitor activity, and patient census to inform business development strategies.
  • Develop and maintain relationships with key stakeholders, including hospital administrators, medical directors, and other healthcare leaders.
