Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Hackensack, New Jersey.

Job Summary:

We are looking for a dedicated Physical Therapist to provide high-quality care to our patients in the Northern Bergen County area. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and performing interventions to improve patient well-being.

Responsibilities:
  • Evaluate patient functional status and physical therapy needs
  • Assist the physician in developing the therapy plan of care
  • Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Requirements:
  • Current Physical Therapy licensure in the state of New Jersey
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
