Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their rehabilitation goals.
Responsibilities:- Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
- Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance activities, to promote clients' physical function and mobility.
- Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care and coordination.
- Provide education and training to clients and their families on proper techniques and strategies for maintaining physical function and preventing complications.
- Document all interactions, assessments, and interventions in a timely and accurate manner.
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
