Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in South Hackensack, New Jersey.

Job Summary:

We are looking for a dedicated Physical Therapist to provide high-quality care to our patients in their homes or alternate community settings. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering compassionate care.

Responsibilities:
  • Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
  • Develop and implement personalized plans of action in collaboration with physicians to achieve optimal patient outcomes.
  • Document all changes in patient condition and update care plans accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to promote patient education and empowerment.
  • Monitor the quality and appropriateness of services provided and adhere to agency infection control and safety policies.
  • Adhere to the organization's policy regarding absenteeism and appearance.
Requirements:
  • Physical Therapy Licensure in NJ is required.
  • 2-3 years of prior clinical experience and community-based experience are preferred.
  • Strong computer skills and excellent communication and interpersonal skills are essential.
  • A valid NJ driver's license is required.
Working Conditions/Physical Demands:

The Physical Therapist will work indoors and outdoors, drive, lift, sit, stand, walk, and use a phone and tablet for documentation.
