Job Summary:We are looking for a dedicated Physical Therapist to provide high-quality care to patients in their homes or alternate community settings. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering compassionate care.
Responsibilities:- Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
- Develop and implement personalized plans of action in collaboration with physicians to ensure optimal patient outcomes.
- Provide skilled physical therapy interventions to patients in their homes or alternate community settings.
- Monitor patient progress, update care plans, and document all changes in patient condition.
- Identify counseling needs and develop effective teaching strategies to support patient education.
- Adhere to agency infection control and safety policies, as well as organizational policies regarding absenteeism and appearance.
- Physical Therapy Licensure in New Jersey is required.
- 2-3 years of prior clinical experience and community-based experience are preferred.
- Strong computer skills and excellent communication and interpersonal skills are essential.
- A valid New Jersey driver's license is required.
