Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Hackensack, New Jersey.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in their homes or alternate community settings.

Responsibilities:
  • Assess and evaluate patients for physical therapy needs and develop individualized plans of action in collaboration with physicians.
  • Provide physical therapy evaluation and treatment to patients, including exercises, stretches, and other interventions to improve mobility and function.
  • Document all changes in patient condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to promote patient education and empowerment.
  • Monitor quality and appropriateness of services provided and adhere to agency infection control and safety policies.
Qualifications:
  • Physical Therapy Licensure in New Jersey required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid New Jersey driver's license.
