Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to provide high-quality care to our clients in their homes.

Job Summary

We are looking for an experienced Physical Therapist to work closely with our multidisciplinary healthcare team to develop and implement individualized care plans that promote the health, safety, and independent living of our clients.

Responsibilities
  • Conduct home visits to assess client needs and develop care plans.
  • Implement evidence-based interventions to promote wound healing, improve mobility, and enhance overall health.
  • Collaborate with nursing and other healthcare professionals to ensure seamless care coordination.
  • Educate clients and their families on rehabilitative care and activities necessary for optimal health.
  • Accurately document client care management and services provided.
Qualifications
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
