About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Providence PACE team in Milwaukie, Oregon. As a Community Care RN, you will provide high-quality, patient-centered care to older adults in their homes and communities.

Key Responsibilities
  • Assess, plan, and implement individualized care plans for patients with complex needs.
  • Collaborate with interdisciplinary teams to coordinate care and services.
  • Provide education and support to patients and their families to promote health and independence.
  • Supervise and delegate tasks to nursing assistants and other caregivers as needed.
Requirements
  • Current Oregon Registered Nurse license.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Relevant certifications applicable to the position, as specified by the Nurse Manager.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • 1 year of experience in home health nursing.
