Job Summary

We are seeking a skilled and empathetic Registered Nurse to join our team. As a Nurse Care Specialist, you will play a vital role in providing high-quality care to our patients, working collaboratively with our interdisciplinary team to ensure seamless care transitions and exceptional patient outcomes.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans, taking into account patient preferences and goals
  • Collaborate with the healthcare team to coordinate care, including physician orders, lab results, and medication management
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices
Qualifications
  • Current Registered Nurse licensure in the state of West Virginia
  • Minimum 1 year of experience in a post-acute care setting
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Ability to lift 25 pounds, push/pull 50 pounds, and stand for long periods
