Stonerise Healthcare is actively seeking a dedicated Mobility Support Specialist to enhance our team.

MISSION STATEMENT:

The role of the Mobility Support Specialist is to facilitate safe transportation for residents to and from internal and external appointments and events, adhering to established protocols. This position requires effective collaboration with team members to ensure that tasks are completed and quality care is provided, fostering a supportive environment and celebrating collective achievements. While prioritizing exceptional resident care, the role also involves managing available resources efficiently.

KEY RESPONSIBILITIES:
  • Safely transport and assist residents to external medical appointments using facility vehicles.
  • Support residents in navigating within the facility for scheduled specialty consultations, such as podiatry, audiology, optometry, and dental services.
  • Assist in transporting residents to and from organized in-house activities.
  • Provide individualized monitoring for residents identified as at risk of elopement during external visits.
  • Perform basic nursing care tasks for residents as necessary.
  • Ensure a clean, comfortable, and secure environment for residents, addressing their needs effectively.
  • Maintain accurate resident documentation records.
  • Adhere to established departmental, center, and corporate policies and procedures, including objectives, quality improvement initiatives, and safety standards.
  • Participate in scheduled training, educational sessions, and orientation programs to maintain certification and enhance care quality.
  • Carry out additional related tasks as assigned or requested.
QUALIFICATIONS & EXPERIENCE:
  • High school diploma or G.E.D. equivalent.
  • Valid Driver's License in the state of employment.
  • Completion of an approved Nurse Aide Training Program or eligibility to enroll and complete such a program within a specified timeframe.
  • Possession of a Red Cross First Aid Certificate.
  • Experience in roles requiring independent decision-making.
  • Previous experience in healthcare or service-oriented industries is preferred.
  • A compassionate and enthusiastic demeanor.
