Job Summary

The Chronic Care Case Manager is a key member of the Hattiesburg Clinic team, responsible for utilizing a collaborative process of assessment, planning, facilitation, and advocacy to meet the health needs of Medicare patients. This role involves communication and targeted intervention to promote optimal, cost-effective outcomes in a clinic setting.

Key Responsibilities
  • Assist Medicare patients in the administration of their health plan benefits
  • Arrange or coordinate services that patients need to get well or stay well
  • Remove barriers that prevent patients from engaging in an appropriate plan of care
  • Work independently and collaboratively within a multi-specialty healthcare environment
Requirements
  • Graduate of an accredited school of nursing
  • Current state licensure as a Registered Nurse
  • At least three years of nursing experience
  • Knowledge of EMR, medical management, and policies and procedures
  • Intermediate skills in word processing, spreadsheet, and database development and maintenance
  • Excellent organizational, written, and oral communication skills
Work Schedule

Monday-Friday, 8:00am-5:00pm
