Job Summary

We're seeking a skilled Nurse Practitioner In-Home Assessment to join our team. As a key member of our care team, you'll be responsible for performing Health Risk Assessments and transitional care for patients discharged from hospitals to prevent re-admissions.

Key Responsibilities

  • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions, as well as determine physical, mental, and social needs
  • Educate and counsel patients and families based on assessment results, including recommendations
  • Perform follow-up visits in transitional care

Requirements

  • Current RN and NP licensure in the state of practice
  • Current CPR certification
  • Enrollment in both Medicare & Medicaid
