Advanced Practice Nurse in Palliative Care

Position Overview:
The Nurse Practitioner will be required to hold a valid license in the State of Mississippi and must adhere to a protocol sanctioned by a licensed physician in Mississippi, along with the approval of the Board of Trustees. This role operates under the guidance of the Executive Director of Post Acute Care and the Chief Medical Officer, collaborating closely with the Medical Director of Palliative Care.
Key Responsibilities:
- Deliver palliative care aimed at managing symptoms and facilitate discussions about care goals to ensure patients and families comprehend their health status, treatment alternatives, and care objectives.
- Engage with the Medical Director of Palliative Care regarding the management of assigned patients.
- Maintain communication with physicians and other healthcare providers about patient care needs and issues.
- Collaborate with discharge planning teams to improve the coordination of palliative care following discharge.
- Participate in weekly Interdisciplinary Team Meetings, Palliative Care Committee meetings, and other relevant gatherings.
- Work cooperatively with other staff to achieve the goals of palliative care and foster positive relationships within the organization and the community.
- Act as a unified representative of the palliative care program when interacting with medical and hospital personnel.
- Contribute to and/or lead educational initiatives for hospital staff regarding palliative care practices.
- Uphold patient rights and confidentiality, adhering to the organization's core values.
- Exhibit the necessary knowledge and skills to effectively communicate and engage with patients, families, and visitors of diverse age groups, while being respectful of their cultural and religious beliefs.
- Perform additional duties as required.
Performance Expectations:
Expectations for performance will be discussed during the orientation and preceptor phases of the role.
- Demonstrate consistent knowledge and proficiency in delivering palliative care to patients.
- Exhibit the ability to collaborate effectively with interdisciplinary team members in patient care.
- Show capability in working with interdisciplinary teams and external organizations to minimize care gaps.
- Provide education and training to hospital staff on palliative care concepts.
- Manage multiple tasks efficiently and prioritize responsibilities effectively.
- Demonstrate leadership qualities, including vision, self-motivation, performance, determination, communication skills, credibility, ethical behavior, and the ability to inspire and achieve results through collaboration.
Qualifications:
Education and Skills:
A graduate of an accredited non-online RN program is required, along with a Master's Degree in Nursing.
Work Experience:
Prior experience as a Nurse Practitioner is preferred, with a minimum of 720 clinical hours required. Experience in Palliative Care, Gerontology, Acute Care, or Oncology is advantageous.
Certification and Licensure:
- Must be licensed as an APRN by the Mississippi State Board of Nursing.
- Certification as a Family Nurse Practitioner by the American Nurse Credentialing Center (ANCC) is mandatory.
Additional Certification:
- Certification in Palliative Care-Advanced Certified Hospice and Palliative Nurse (ACHPN) is required within the first year of employment.
Mental Demands:
Exceptional interpersonal skills are essential. The candidate must consistently demonstrate the ability to work as part of a team, cooperate with colleagues, follow directions, show initiative, and perform effectively under pressure.
