SUMMARY OF RESPONSIBILITIES

This role is essential in connecting individuals experiencing homelessness with housing solutions through temporary financial assistance and supportive services across various counties. The Case Manager will assess client needs, verify eligibility, assist in locating and securing housing, and provide comprehensive case management and supportive services utilizing the Housing First model.

Responsibilities at this level include intensive case management. The position requires specific educational qualifications, along with strong decision-making, problem-solving, and interpersonal skills. A high level of social work expertise, practice, and knowledge is essential.

Compensation: Starting at $19.74 per hour, with potential increases based on relevant experience.

Work Schedule: 37.5 hours per week.

MINIMUM QUALIFICATIONS

  • Bachelor's Degree with two years of direct care experience in Human Services, or an equivalent of six years of case management experience in human services.
  • Valid Driver's License with an acceptable driving record.

Key Duties and Responsibilities

  • *Engage in outreach efforts to connect with homeless individuals, facilitating their access to services and screening for program eligibility.
  • *Conduct interviews and assessments to determine eligibility and make appropriate referrals.
  • *Verify and document applicant eligibility, maintaining accurate service records for all clients.
  • *Assist clients in identifying their strengths, needs, and goals, developing individualized housing stability plans collaboratively.
  • *Support individuals or families in finding, obtaining, and maintaining suitable housing through comprehensive housing search and placement services.
  • *Provide case management services focused on addressing the housing needs of program participants and promoting housing stability.
  • *Assist clients with essential skills such as budgeting, money management, accessing personal credit reports, and resolving credit issues.
  • *Offer problem-solving and crisis intervention services to empower clients towards self-sufficiency.
  • *Conduct work in various settings, including client homes and shelters, ensuring safety and security measures are adhered to.
  • *Travel is required, including the use of a personal vehicle.
  • *Ensure timely collection and reporting of required information in accordance with federal funding and internal guidelines.
  • *Provide program information and referral services to clients and other service providers.
  • *Demonstrate knowledge of community resources and Heartland Family Service programs to effectively meet client needs.
  • *Utilize community resources to assist clients in achieving self-sufficiency.
  • *Evaluate the efficiency and effectiveness of community service providers to ensure quality care for clients.
  • *Work independently with minimal supervision as well as collaboratively within a team.
  • *Maintain regular attendance and punctuality, ensuring accuracy in reports and statistics.
  • *Exhibit a courteous and compassionate demeanor towards clients, volunteers, and visitors.
  • *Adhere to all program and agency policies and procedures.
  • *Collaborate with program staff, volunteers, and other agency personnel.
  • *Evaluate program services and provide recommendations for improvement.
  • *Engage in professional development through training opportunities.
  • *Foster positive relationships with clients, referral sources, service agencies, and landlords.
  • *Create and maintain a personal self-care plan.
  • *Strive to connect the agency with the broader community to support ongoing fundraising and outreach efforts.
  • *Perform other program-related duties as assigned.
