POSITION SUMMARY

This role is focused on connecting individuals experiencing homelessness with housing solutions through temporary financial assistance and supportive services across various counties. The Case Manager will assess client needs, verify eligibility, assist in locating and securing housing, and provide comprehensive case management and supportive services using the Housing First model.

Responsibilities at this level include intensive case management. The role requires specific educational qualifications, strong decision-making abilities, problem-solving skills, and advanced interpersonal capabilities. High-level social work skills, practice, and knowledge are essential for success in this position.

Compensation: Starting at $19.74 per hour, with potential increases based on relevant experience.

Work Schedule: 37.5 hours per week.

MINIMUM QUALIFICATIONS

  • Bachelor's Degree with two years of direct care experience in Human Services, or a total of six years of case management experience in human services.
  • Valid Driver's License with an acceptable driving record.

Key Responsibilities

  • *Engage in outreach efforts to connect with homeless individuals, facilitating their access to services and screening for program eligibility.
  • *Conduct interviews and assessments to determine eligibility and make referrals as necessary.
  • *Verify and document the eligibility of applicants, maintaining comprehensive and accurate service records for all clients.
  • *Assist clients in identifying their strengths, needs, and goals, developing personalized housing stability plans in collaboration with them, and monitoring their progress.
  • *Support individuals or families in finding, obtaining, and maintaining suitable housing through effective housing search and placement services.
  • *Provide case management services to address the housing needs of program participants and help them achieve housing stability.
  • *Assist clients with essential skills related to budgeting, money management, accessing personal credit reports, and resolving credit issues.
  • *Offer problem-solving and crisis intervention services to empower clients towards self-sufficiency.
  • *Conduct work in various settings, including client homes and shelters, while ensuring safety and security protocols are followed.
  • *Travel is required, including the use of a personal vehicle.
  • *Ensure timely collection and reporting of required information in accordance with federal funding and internal guidelines.
  • *Provide program information and referral services to clients and service providers.
  • *Demonstrate knowledge of community resources and Heartland Family Service programs to effectively meet client needs.
  • *Evaluate the efficiency and effectiveness of community service providers to ensure quality care for clients.
  • *Work independently with minimal supervision and collaboratively as part of a team.
  • *Maintain regular attendance and accuracy in scheduling and reporting.
  • *Exhibit a courteous and compassionate demeanor towards clients, volunteers, and visitors.
  • *Adhere to all program and agency policies and procedures.
  • *Collaborate with program staff, volunteers, and other agency personnel.
