ROLE SUMMARY
This role involves connecting individuals facing homelessness with housing solutions through temporary financial assistance and supportive services across various counties. The Case Manager will assess and verify eligibility, assist clients in finding and securing housing, and provide case management and supportive services utilizing the Housing First model.
Responsibilities at this level include intensive case management. Educational qualifications are specific, and the role demands strong decision-making, problem-solving, and advanced interpersonal skills. A high level of social work expertise, practice, and knowledge is essential.
Compensation: Starting at $19.74 per hour, with increases based on relevant experience.
Work Schedule: 37.5 hours per week.
Key Qualifications
- Bachelor's Degree with two years of direct care experience in Human Services, or a total of six years of case management experience in human services.
- Valid Driver's License with a satisfactory driving record.
Core Responsibilities
- Engage with homeless individuals through outreach to connect them with services and assess eligibility for programs.
- Conduct interviews and assessments to determine acceptance, denial, or referral of applicants.
- Verify and document applicant eligibility while maintaining comprehensive and accurate service records for all clients.
- Assist clients in identifying their strengths, needs, and goals, and develop individualized housing stability plans collaboratively.
- Support individuals or families in locating, obtaining, and retaining suitable housing through targeted search and placement services.
- Provide case management services to address the housing needs of program participants and facilitate housing stability.
- Guide program participants in essential skills such as budgeting, money management, and resolving credit issues.
- Offer problem-solving and crisis intervention services to empower clients towards self-sufficiency.
- Conduct work in various settings, including client homes and shelters, ensuring safety and security protocols are followed.
- Travel is required, including the use of a personal vehicle.
- Ensure timely collection and reporting of required information as per federal funding and internal guidelines.
- Provide program information and referral services to clients and other service providers.
- Demonstrate knowledge of community resources and Heartland Family Service programs to effectively meet client needs.
- Evaluate the efficiency and effectiveness of community service providers to ensure quality care for clients.
- Work independently with minimal supervision as well as collaboratively within a team.
- Maintain regular attendance and punctuality, ensuring accuracy in reports and statistics.
- Exhibit a courteous and compassionate demeanor towards clients, volunteers, and visitors.
- Adhere to all program and agency policies and procedures.
- Collaborate with program staff, volunteers, and other agency personnel.
- Engage in continuous skill development through training opportunities.
- Build positive relationships with clients, referral sources, service agencies, and landlords.
- Create and maintain a dynamic self-care plan.
- Contribute to the agency's fundraising and community engagement efforts.
- Perform other related duties as assigned.
