Job Title: Hospice Registered Nurse Case Manager

As a Hospice Registered Nurse Case Manager, you will play a vital role in providing exceptional care to patients and families in Charleston, Berkeley, and Dorchester Counties.

Job Summary

We are seeking a compassionate and skilled RN to join our team as a Hospice Case Manager. In this role, you will be responsible for maintaining a specified case management load, completing initial and ongoing assessments of patients and families, and developing and implementing individualized plans of care.

Responsibilities
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct initial and ongoing assessments of patients and families to determine hospice needs, including physical and psychosocial evaluations.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
  • Direct and supervise the services provided by and assigned to patients and families.
  • Facilitate the consideration of alternative levels of care when indicated by patients' and families' status and needs.
Requirements
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
  • If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
  • One (1) year of hospice care experience.
  • One (1) year of precepting or training experience preferred.
  • CPR certification.
  • Able to provide proof of valid driver's license and auto liability insurance.
