Job Description

As a Hospice RN Case Manager at Interim HealthCare Staffing, you will play a vital role in providing exceptional care to patients and their families. Your expertise and compassion will make a significant difference in the lives of those we serve.

Key Responsibilities:

  • Maintain a specified case management load, ensuring timely and effective care delivery.
  • Conduct comprehensive assessments of patients and their families to determine hospice needs, including pain and symptom management.
  • Participate in on-call duties as defined by the office on-call policy, providing 24/7 support to patients and families.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and external healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, as indicated by patient status and needs.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and a valid driver's license with auto liability insurance.
