Hospice RN Case Manager Job Description

As a Hospice RN Case Manager with Interim HealthCare, you will play a vital role in providing compassionate and expert care to patients and their families. Your primary responsibility will be to maintain a specified case management load, ensuring that each patient receives the highest level of care and support.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and patients' families
  • Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services
  • Supervise and direct services provided by LPNs, Hospice Aides, and Homemakers
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in South Carolina
  • 12 months of accumulated experience as a registered nurse within the last 5 years or evidence of current clinical knowledge
  • 1 year of hospice care experience and 1 year of precepting or training experience preferred
  • CPR certification and valid driver's license
