Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide exceptional care to patients and families, upholding our high standards of nursing practice.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct comprehensive assessments of patients and families, identifying their unique needs and developing personalized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care delivery and achieve the goals of hospice care.
  • Coordinate services among IDG members and external healthcare providers to meet the evolving needs of patients and families.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
  • Evaluate and coordinate the need for services, ensuring alignment with the IDG plan of care.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
