As a Hospice RN Case Manager with Interim Healthcare Staffing, you will play a vital role in delivering compassionate and skilled hospice care to patients and families. Your expertise will be essential in assessing patient needs, developing personalized care plans, and coordinating services to ensure seamless care transitions.

Key Responsibilities:

  • Conduct thorough patient assessments to identify care needs and develop effective care plans
