Job Summary

We are seeking a highly organized and detail-oriented Team Coordinator to join our LifeCare Home Health team. As a Team Coordinator, you will provide administrative support to our Clinical Manager and play a critical role in ensuring the smooth operation of our team.

Key Responsibilities
  • Administrative Support: Provide clerical support to the Clinical Manager, including managing day-to-day team operations, tracking systems, and communication with staff.
  • Workflow Management: Monitor assigned reports, follow up on unresolved discrepancies, and maintain accurate and up-to-date information in tracking systems.
  • Communication: Distribute and/or provide communications to appropriate staff, receive, address, triage, and relay messages for assigned Team field staff.
  • Quality Management: Participate in quality management activities as assigned and facilitate maintaining, updating, and communicating visit schedules for Team field staff.
  • Customer Service: Support Agency's customer service reputation by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts.
Requirements
  • Education: High school diploma and experience in computer processing preferred.
  • Skills: Above average word processing/data entry skills, knowledge of medical terminology, excellent communication skills, and ability to maintain confidential information.
Working Conditions
  • Physical Demands: Sitting, standing, and walking are required. Ability to handle stressful situations in a calm and courteous manner at all times.
  • Environmental Conditions: Works under a variety of conditions in facilities and offices. Available to agency personnel in person or by telephone during the Agency operating hours and possibly after hours for emergencies.
