About the Role

We are seeking a highly motivated and compassionate Family Liaison to join our team at Horizon Health Services. As a Family Liaison, you will play a critical role in supporting families and adolescents in our community, providing guidance and resources to help them navigate the healthcare system.

Key Responsibilities
  • Develop and maintain relationships with families and adolescents, providing emotional support and guidance to help them achieve their goals.
  • Collaborate with healthcare providers and community organizations to ensure seamless care coordination and access to resources.
  • Provide education and training to families and adolescents on healthy behaviors, mental health, and wellness.
  • Conduct home visits and community outreach to engage with families and adolescents in their natural environments.
  • Maintain accurate and timely records of client interactions and progress.

Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a related field, such as social work, psychology, or education.
  • At least 2 years of experience working with families and adolescents in a healthcare or social services setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
