Key Responsibilities- Pre-screening of applicants or referrals per established guidelines and policy
- Assessing applicants for appropriate placement
- Conducting pre-admission interviews of applicants
- Obtaining necessary documentation for admission
- Submitting statistical monthly reports to the direct supervisor
- Baccalaureate Degree in Social Work, R.N., or related field from an accredited institution
- 3-5 years of experience in hospital patient admissions or related field
- Strong assessment skills and working knowledge of medical charts and terminology
- Valid driver's license
