Job Summary:
We are seeking a Hospice Registered Nurse to join our team in Beaumont, TX. As a Hospice Nurse, you will provide high-quality patient care, develop and implement individualized plans of care, and collaborate with interdisciplinary teams to ensure seamless delivery of services.
Responsibilities:
- Assist in the development, review, and revision of patient care plans
- Provide nursing care in accordance with physician orders and individualized patient plans
- Observe, record, and report any changes in patient condition to appropriate personnel or physicians
- Provide education and supervision to patients, families, or other healthcare team members as needed
- Participate in quality assessment activities, staff meetings, and education programs
Requirements:
- RN License in the state of Texas
- Home Health experience preferred
- Strong organizational and self-management skills
- Strong customer service skills
- Valid driver's license
