We are seeking a dedicated Hospice RN / RN Case Manager to join our team at Gentiva, a leading provider of hospice and palliative care services. As a key member of our interdisciplinary team, you will play a vital role in delivering compassionate patient-centered care to individuals and their families.

About the Role

  • You will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of each individual.
  • As a seasoned RN Case Manager, you will possess excellent communication and interpersonal skills, ensuring seamless collaboration among team members and effective communication with patients and families.
  • Your expertise in nursing and case management will enable you to assess patient needs, identify opportunities for improvement, and implement evidence-based practices to enhance patient outcomes.

What We Offer

  • A competitive salary of approximately $65,000 - $85,000 per year, commensurate with experience and location.

Requirements and Qualifications

  • A current RN license in the state of employment (or eligibility to obtain).
  • At least one year of experience in hospice, home health, ICU, geriatric, or oncology nursing.
  • Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
