Job Summary

We are seeking a compassionate and skilled Hospice Nurse Practitioner to join our team at Concho Hearts Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families navigating end-of-life illnesses.

Key Responsibilities
  • Under the supervision of a physician, conduct clinical examinations of patients, perform diagnoses, and coordinate care with the interdisciplinary team.
  • Provide medical care, palliation, and management of terminal illnesses in compliance with state regulations.
  • Conduct history and physical examinations, evaluating patients' medical conditions and managing health issues.
  • Complete face-to-face visits, ensuring compliance with regulatory requirements.
  • Provide clinical information to the hospice medical director for interpretation and eligibility determination.
Requirements
  • Licensed Nurse Practitioner with a valid State Nurse Practice Act license, required.
  • Minimum one year of experience as a Nurse Practitioner, preferred six months to one year in hospice or pain and symptom management.
  • Proven decision-making skills, excellent interpersonal abilities, and effective communication skills.
Compensation and Benefits

We offer a competitive salary range of $120,000 - $150,000 annually, depending on experience. Additionally, we provide a comprehensive benefits package, including auto liability insurance, paid time off, and opportunities for professional growth and development.

Work Environment

You will work in patients' homes, occasionally encountering variable conditions, exposure to blood and bodily fluids, and infectious diseases. A flexible schedule and ability to travel locally are essential. Some exposure to unpleasant weather and PRN emergency calls may be required.
