At Concho Hearts Hospice, we are seeking a compassionate Licensed Medical Social Worker (LMSW or LBSW) to join our team as a PRN Hospice Social Worker. This is a rewarding opportunity for individuals who want to make a meaningful difference in the lives of patients and their families.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work collaboratively with interdisciplinary teams. They will be responsible for providing social services to patients and their families, including assessing psychosocial status, developing individualized plans of care, and counseling on coping difficulties.

We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, as well as an industry-leading benefits package that includes time-off package, mileage reimbursement, comprehensive health, dental, and life insurance coverage.

To be successful in this role, you will need to have a Master's of Social Work from an accredited school of social work or Bachelor of Social Work, hold a current unencumbered license of Social Work in Texas, and have at least one year of experience as a social worker in long-term care or medical-surgical/acute care setting. Prior hospice experience is preferred but not required.

As a PRN Hospice Social Worker, you will be working flexible hours and traveling between offices, facilities, etc. You must be computer proficient in typing and various programs, including a background in EMR, and possess the ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death, and grief.
