Job Summary

The SCO Family of Services is seeking a highly skilled and experienced Care Management Supervisor to oversee the daily operations of our Care Management Program. As a key member of our team, you will be responsible for providing administrative oversight to a team of Care Managers, ensuring the delivery of high-quality care to children, adults, and families with complex needs.

Key Responsibilities
  • Oversee the identification of eligible Health Home members and facilitate the program enrollment and consent process.
  • Provide weekly/bi-weekly administrative field supervision to a unit or up to 8 individual Care Managers.
  • Develop and nurture leadership qualities in all line staff by troubleshooting administrative issues.
  • Monitor and review all Care Management required documentation, and track all cases to ensure compliance with governing bodies.
  • Supervisors must attend forums/meetings and participate in conference calls as needed, and accompany the Care Manager on field visits to facilitate case discussions and provide additional supervision in the field as needed.
  • Provide staff with adequate compliance support, including documentation, case tracking, and interdisciplinary team meetings.
  • Review electronic case records and/or personnel files during supervision to ensure compliance.
  • Provide effective performance feedback through employee recognition, rewards, and disciplinary action, with the assistance of Human Resources, when necessary.
  • Maintain Care Managers work schedules, including assignments, and provide case coverage when applicable, both internal and external.
  • Provide direct Care Management services in the absence of Care Management staff.
  • Coordinate and ensure execution of plans for audit preparation.
  • Review and ensure that electronic Health record is up to date and required documentation is current and uploaded.
  • Provide information regarding audit readiness, including Key Performance Indicators (KPI) data, data regarding unit case records, and number of visits completed minimally on a monthly basis to the Directors.
Requirements
  • Master's degree preferred, with a B.A. Degree in Social Work and 4 years of experience in the child welfare field, or a qualified Health Care Practitioner with 2-5 years of administrative, clinical, and supervisory/managerial experience.
  • Knowledge of best practice working with children and families in health, preventive, and wellness care.
  • Passion and knowledge for advocating on behalf of staff, children, and families.
  • Demonstrates professionalism at all times, maintains workplace boundaries, promotes a productive and positive work atmosphere, upholds agency/program vision and mission, and adheres to agency Code of Conduct.
  • Strong team player with excellent communication and leadership skills, able to work well with others and provide direction and guidance to subordinates.
  • Evidence of the ability to practice a high level of confidentiality, develop a trusting and supportive relationship with subordinates, and have a strong sensitivity to cultural differences presented among staff and clients within the agency.
  • Demonstrates excellent crisis intervention skills and the ability to remain calm while handling crisis situations.
