Position Summary:

Under the guidance of the Director of Transitional Care, the Patient Care Coordinator is responsible for managing a specific patient caseload. This role involves effective planning to address patient needs, optimizing hospital stay durations, and ensuring the efficient use of resources. The case management process is collaborative, encompassing assessment, planning, implementation, coordination, monitoring, and evaluation of services to meet patients' health and human service needs. This role emphasizes advocacy, communication, and resource management, aiming for quality and cost-effective outcomes in line with regulatory standards.

Qualifications:

Education: Must possess a degree from an accredited nursing program, either an Associate's or Bachelor's degree in nursing.

Training: Demonstrated management capabilities with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and effective communication.

Licensure: Must hold and maintain a valid Registered Nurse license in the State of Georgia or a compact license, along with current BLS CPR certification.

Experience: A minimum of two years of experience in acute hospital nursing is required; two years of experience in acute hospital case management is preferred. Familiarity with utilization review processes and guidelines is advantageous, as are intermediate computer skills in word processing and spreadsheets.

Performance Standards:

  1. Conduct admission and continued stay reviews utilizing evidence-based clinical guidelines.
  2. Engage with insurance providers to present clinical data supporting admissions and ongoing hospital stays.
  3. Collaborate with the Physician Advisor for case reviews and data collection as directed.
  4. Act as a vital member of a multidisciplinary team, adjusting care plans based on patient conditions.
  5. Provide necessary federal notices to Medicare beneficiaries in accordance with guidelines.
  6. Support nursing and ancillary services in maintaining quality care delivery.
  7. Advocate for patients regarding the appropriate use of benefits and community resources.
  8. Complete clinical reviews in a timely manner based on medical necessity assessments.
  9. Understand medical necessity and service intensity, integrating payer requirements into discharge planning.
  10. Consider readmission risks and socio-economic factors in transition planning.
  11. Involve patients and their support networks in the transition planning process.
  12. Maintain communication with the Transitional Care Director regarding patient care and outcomes.
  13. Facilitate patient care conferences and family meetings.
  14. Encourage peer discussions among healthcare professionals regarding medical necessity evaluations.
  15. Report any medical/legal issues promptly to the appropriate parties.
  16. Foster positive relationships with other departments and community health agencies to meet patient needs.
  17. Continuously assess personal competencies to ensure effective job performance.
  18. Adhere to infection control, OSHA, and safety standards.
  19. Perform additional duties as required.
